Graduate Program Forms

Listed below are the various forms relevant to the graduate program in computer science. These are all PDF documents, and you will need the appropriate software to display or print them. Various other graduate forms are available from the graduate college.

Independent Study Form

This form is used to propose a topic for a graduate independent study course (CSCI 8970). It requires identification of the faculty who will supervise the independent study, and should be prepared in consultation with that person. Approval of the independent study by the graduate program committee is required prior to registration.

Internship Form

This form apparently does not exist yet.

Graduate Assistantship Application

Complete this form to apply for a graduate assistantship.

Proposed Supervisory Committee

This form is used to identify the proposed members of a thesis or project supervisory committee. It requires the names and signatures of the proposed members.

Thesis Proposal Approval

This is the form used by a thesis supervisory committee to indicate their approval of a proposed thesis topic. It requires the names and signatures of the proposed members.

Project Proposal Approval

This is the form used by a project supervisory committee to indicate their approval of a proposed project topic. It requires the names and signatures of the proposed members.
